When you are preparing for the lesson, you may want to print this form as a template for capturing the information you will subsequently enter on the web site. Each box on this form corresponds to values on the class worksheet.

Important note: If you have trouble submitting the form (you get a server not responding error, or pipe broken error), try again in about an hour or two. Remember, this is the internet, and world-wide traffic can have an impact on your performance. If you still can't get through, copy the contents of this page to the clipboard, and paste it into your e-mail program. Complete the data and mail it to mcornell@uiuc.edu.

Section 1: General Information This information will be used to catalog your results. The state or country will determine in which "region" your results will be included. If you have more than 20 students, please submit multiple forms. Since the data is analyzed on a regional basis only, the number of forms you submit for your class (or classes) will not effect the analysis. Please note: If you submit more than one form, please change the group number entry below.

Section 2: Student Information Provide the total number of boxes (pens) purchased by each student. These totals can be found in the right-hand column of the Class Worksheet.

Remember, If you have additional students, please submit another form. Leave all information the same, except increment the Group Number field.

Section 3: Pen Color Totals Provide the total number of pens rolled for the students listed in Section 2. These totals can be found in the second-to-last row of the Class Worksheet. If you are submitting multiple forms, do not put the pen totals for all of your students on one form. Please break out the totals by each group of 20 (or fewer) students. As a check, the total of Section 3 should equal the total of Section 2.
